You can add as many fields as you wish to a form. There are three different types of fields to choose from: Fields already created in Upsales, Opt-in & Terms, and Form specific fields.

Fields already created in Upsales
These fields refer to existing fields on the Company and Contact Card in Upsales.
If the company and/or contact is new, they'll be created in Upsales and the information provided in the form will be added to them.
If the company and/or contact exists in your database, then any new information will be added to their cards.
Opt-in & terms
If you wish to add opt-in options for GDPR terms and other terms and agreements, these are the fields to use. You can add new Opt-in terms in administrator Settings > Marketing > Opt- in and terms.
Form specific fields
Fields containing information that will only be visible through form submits. Meaning, it will not be saved to the Company or Contact card in Upsales.
Adding fields to a form
Add Fields already created in Upsales
- Click Add field
- Choose Fields already created in Upsales

Scroll through Company fields or Contact fields, or search for a field if you know what you're looking for.
- Once you've found the field you're looking for, click on it to add to the form.
- Add any additional fields you wish, then click the Back button to continue editing.
- Don't forget to give the field an appropriate name.
Add Opt-in & terms fields
- Click Add field
- Choose Opt-in & terms
- Choose among available Opt-in templates. To create new templates, go to administrator Settings > Marketing > Opt-in & Terms.
- Clicking on an Opt-in template will add it to the form. Don't forget to make it required if needed, by clicking Required toggle.
Add Form specific fields
- Click Add field
- Choose Form specific fields
- Click on the type of field you wish to add. The information collected from these fields will only show in form submits, and will not be added to contact or companies.
- Once you've clicked on a type of field, it will be added to your form. Be sure to name the field appropriately, depending on what type of field you've added.
